Purpose
This policy outlines the maternity leave provisions available to employees at ________, ensuring compliance with applicable US federal and state laws and promoting the well-being of employees and their families during pregnancy, childbirth, and the postpartum period.
Scope
This policy applies to all eligible full-time and part-time employees who are expecting a child through birth, adoption, or surrogacy, and meet the eligibility requirements below.
Eligibility
- Employed for at least 12 months
- Worked a minimum of 1,250 hours during the 12 months preceding the leave
- Works at a location where at least 50 employees are employed within a 75-mile radius, in accordance with FMLA
Duration of Leave

Paid Leave Options
